Asthma Care

For patients who are due an annual asthma review.

Please would you answer the questions on the form below and submit it to us.

If your symptoms are deteriorating or you have any concerns, please make an appointment to the respiratory nurse or a doctor as well.

Asthma Care for your Child

We are committed to working in partnership with parents in order to promote high quality asthma management for children registered at the practice.

Your child will be invited to a 30 minute asthma review appointment at least every 6 months or more frequently if your child is experiencing any problems with their asthma.

Your child's asthma review will include a basic lung function test for children over the age of 6, an inhaler technique check and completion of an asthma control test questionnaire. You will have the opportunity to discuss any concerns you may have regarding your child's asthma. You will also be offered a written asthma management plan for your child in order for you to feel more confident at managing your child's asthma at home. Additionally your child will be offered registration on a new Pennine care interactive asthma support website (www.puffin.penninecare.nhs.uk)

In the event of your child experiencing an acute asthma flare up, your will always be offered a same day urgent GP appointment if required.
